To enable treatment to be performed by Fur Ability Animal Rehabilitation we require a signed referral form from your regular veterinarian with full medical records and any diagnostics that have been performed.
Once a referral has been received we also require a completed new patient form. This gives us extra information on how you feel your pet is doing and your expectations and goals of treatment.

This form is for referral for rehabilitation treatment. It must be completed by the regular veterinarian of the pet. Fur Ability is unable to diagnose or perform diagnostics so we as that these take place before referral.
If you are the referring veterinarian and the diagnosis is unclear please contact Fur Ability before submitting this form. We may still be able to provide treatment with your consent and any findings will be reported back to enable a formal diagnosis.
Appointment findings and medical notes will be regularly communicated back to the referring veterinarian via email unless indicated otherwise
This form is for referral ongoing maintenance rehabilitation for healthy stable patients. It is not intended as a referral for a specific condition but rather a for patients to be able to have regular rehabilitation treatments to ensure they are in peak condition.
It must be completed by the regular veterinarian of the pet. The purpose of this form is to ensure there are no contraindications to maintenance treatments taking place.
Appointment findings and medical notes will be regularly communicated back to the referring veterinarian via email unless indicated otherwise. If the pets condition changes or a new problem is found the patient will be required to seek veterinary treatment and if necessary a full referral for treatment of the new diagnosis may be required.
This form is for referral for therapeutic laser of a specific condition. It must be completed by the regular veterinarian of the pet.
Please Note: Laser therapy is beneficial for conditions such as pain, tendon and ligament injuries, muscle injuries, nervous system injury and neurologic conditions and osteoarthritis. However, a referral for a full rehabilitation consultation is required to treat these issues/conditions. This is due to Fur Ability wishing to provide gold standard care to our patients and we use therapeutic laser alongside other techniques for the best outcome for the patient in these cases.

This form is to completed by the owner. It is used to gather more information about you, your pet, your views of how they are doing. As well as information about your goals and expectations of rehabilitation. This helps alongside your vets medical notes and our findings in the initial consultation to design your pets individual treatment plan designed to work for you, your pet and your lifestyle!
