Start Your Pet’s Rehabilitation Journey
To begin treatment with Fur Ability, we require two forms:
- A referral form completed by your regular veterinarian, including medical history and diagnostic records
- A new patient form completed by the pet owner
These forms ensure we provide safe, effective, and individualised care. If you’re unsure which form applies, please contact us and we’ll be happy to help.
For Veterinarians
General Rehabilitation Referral Form
Required for patients referred for treatment of a specific medical condition. Please include all relevant diagnostics and clinical notes.
Therapeutic Laser Referral Form
For patients referred specifically for laser therapy. Please note that a full rehabilitation referral is needed for complex or chronic cases.
For Pet Owners
New Patient Information Form
This form helps us understand your pet’s daily function, medical history, and your goals for their care. It complements the medical referral and allows us to tailor the best plan possible.
